1945 Burbank Parks and Recreation Survey

On March 14, 1945, the Burbank Parks and Recreation Department handed out historical data forms for people to fill out. These forms were collected and, somehow over the years, made their way to the Burbank Historical Society. When I learned about these forms by reading Susie Hodgson's article "1945 - Survey Says!", I thought it would be nice to get these forms and make a HTML transcription, so they may be made searchable on the Internet. I asked Susie, Susie asked others in the Historical Society, and permission was granted. (Thank you!) The following is a transcription of the 1945 survey forms. An interesting 75 year-old document!
